While Medicare provides essential health coverage for seniors and certain individuals with disabilities, its various parts and options can feel like an alphabet soup of confusion. The Four Parts of Medicare: A Simple Breakdown

  • Medicare Part A (Hospital Insurance): This is often premium-free if you or your spouse worked and paid Medicare taxes for at least 10 years. Part A helps cover inpatient hospital care, skilled nursing facility care, hospice care, and home health care. However, it comes with a significant deductible per benefit period.
  • Medicare Part B (Medical Insurance): Part B covers your day-to-day medical needs. This includes doctor visits, outpatient care, preventive services, medical supplies, and ambulance services. You will pay a monthly premium for Part B, and it also has an annual deductible and 20% coinsurance for most services.

Original Medicare (Parts A & B) is the foundation, but it leaves significant gaps in coverage. That 20% coinsurance has no annual limit, meaning a serious illness could lead to substantial out-of-pocket costs. This is why most people choose to supplement their coverage in one of two ways.

Your Two Main Paths for Enhanced Coverage

Path 1: Medicare Supplement (Medigap) + Part D
A Medicare Supplement plan, also known as Medigap, is a private insurance policy that works alongside Original Medicare. Its purpose is to “fill the gaps” by helping to pay for some or all of the costs that Medicare doesn’t cover, like your Part A deductible and the 20% Part B coinsurance.

  • Key Benefits of Medigap:
    • Predictable Costs: You pay a monthly premium, but your out-of-pocket costs for medical care are significantly reduced or even eliminated.
    • Freedom of Choice: You can see any doctor or visit any hospital in the U.S. that accepts Medicare. No network restrictions or referrals are needed.
    • Standardized Plans: Medigap plans are standardized by letter (e.g., Plan G, Plan N). This means a Plan G from one company has the exact same core benefits as a Plan G from another, allowing you to shop based on price and company reputation.

If you choose this path, you will also need to enroll in a standalone Medicare Part D (Prescription Drug Plan). Part D plans are offered by private insurers and help cover the cost of your medications.

Path 2: Medicare Advantage (Part C)
A Medicare Advantage plan is an all-in-one alternative to Original Medicare. These plans are offered by private insurance companies approved by Medicare. When you enroll in a Medicare Advantage plan, you get your Part A and Part B benefits bundled together, and most plans also include Part D prescription drug coverage.

  • Key Benefits of Medicare Advantage:
    • Low or $0 Monthly Premiums: Many plans have very low or even no additional monthly premium beyond your standard Part B premium.
    • Extra Benefits: These plans often include routine dental, vision, and hearing coverage, as well as gym memberships (like SilverSneakers) and allowances for over-the-counter products—benefits not covered by Original Medicare.
    • Out-of-Pocket Maximum: Every plan has an annual limit on your out-of-pocket costs, providing a crucial financial safety net.

The trade-off is that these plans typically operate with a local provider network (like an HMO or PPO), and you may need referrals to see specialists.
